Tangela (Japanese: モンジャラ Monjara) is a Grass-type Basic Pokémon card. It first appeared as a card exclusive to Pokémon Trading Card Game for the Game Boy Color, but was later released in physical form in Pokémon Web.
Card text
Flip a coin. If heads, the Defending Pokémon is now Paralyzed.
The Defending Pokémon is now Poisoned.

This is only one of three Game Boy-exclusive cards that were later made actual cards. The others are Electrode and Dragonite.
Origin
Stun Spore is a move in the Pokémon games that Tangela can learn. This card's English Pokédex entry comes from Pokémon Yellow, whereas the Japanese entry comes from Pokémon Blue.
