This is the Bulbapedia walkthrough for Pokémon HeartGold and SoulSilver Versions. This walkthrough follows the remade Nintendo DS versions, not Pokémon Gold and Silver.
The guide for those can be found here.
Route 27 (west)
Route 27 is the first area of the Kanto region that you will encounter. After surfing across the western pond, a guy standing nearby will notify you that you have just set foot into the new region. Jump off of the southern shoreline, and surf east to find a Rare Candy. Swim back west and step inside the cave to reach Tohjo Falls.
Tohjo Falls
Tohjo Falls
The Link between
Kanto and Johto
The Tohjo Falls are a pair of waterfalls that links Johto and Kanto, as all Trainers from Johto must pass by on their way to the Pokémon League. Swim out from the entrance to climb the first waterfall, and then ride the eastern wall of water towards the exit. Surf to the east to find a rock-hewn stairway leading to a walkway behind the waterfalls; follow it to the west to find a Moon Stone.
Disguised by the cascading water to the west is a tiny room with nothing of interest but a few breakable rocks and a broken radio. It almost looks like someone was hiding out here...

Route 27 (east)
As you exit Tohjo Falls, visit the old woman in the house on the ledge. If your lead Pokémon's friendship is high enough, she'll reward you with TM37 (Sandstorm). Continue east across the water, and you'll reach a bridge; swim past the whirlpool to the south of Ace Trainer Brian to reach TM02 (Dragon Claw). Walk east from the bridge, and you've got two choices: climb along the cliffside and pick up a Destiny Knot, or simply swim across the whirlpool. Fight past Ace Trainer Reena, and walk up onto the rock outcropping to reach Route 26.

Route 26
Leaving Route 27 from the west, cross the two bridges in order to reach solid ground again. Hike through the two patches of tall grass, and follow the path to the north to find a house with a blue Apricorn tree growing nearby. Speak to the woman inside, and she'll restore your team to full health. If you travel through the large patch of grass to the southwest, you'll reach the home of the Week Siblings; inside is a notebook that lists the day and location that each one appears. Leave the rest stop and continue north on the path, fighting Ace Trainers Joyce, Gaven, Jake, and Jamie as you go; this leads you directly to the Reception Gate. Before you enter the checkpoint, jump down the series of ledges to the west to find a Max Elixir, and loop around to step inside the building.

Pokémon League Reception Gate
As you enter, the first guard will check to see that you have all eight of Johto's badges before allowing you to pass. Beyond him, the Reception Gate is the intersection of three paths; the Kanto region to the east, Mt. Silver to the west, and Victory Road to the north. With the guards blocking the east and west routes, your only option is to head north to Victory Road.
Victory Road
Victory Road
Victory Road is a rough and rocky place, a reminder that the challenges ahead of you won't be easy either. There are three floors in all, and in some places you'll have to jump down through holes in the floor to get where you need to go.

1F
Walk north from the Reception Gate's northern exit, and shove the boulder out of the way. Turn west, step down the stairway, and follow the path to the east. Pick up the Potion, climb the stairway, and cross the rope bridge to the north. Climb down the stairway to the west, and pick up the Full Heal sitting under the bridge. Continue to the north, and navigate through the maze of rocks to the right to find a Max Revive. Climb the ladder north of here to reach the second floor.
2F
Head south and climb up onto the lower rock ledge. Walk north to find a Max Revive hidden on a lone rock, and continue east. Take the ladder to the east to reach 3F.
3F
Use your Dowsing MCHN to find a Hyper Potion, and jump down the hole in the floor to reach the eastern side of 2F.
2F
Walk north from where you landed, and sidestep around the single boulder to find a Full Restore. Climb the ladder in the northeast to return to the third floor.
3F
Hike west past the hole in the floor, and step up onto the rock ledge. Walk to the north to find an Ultra Ball, and continue west to find two more holes in the floor; drop through the northern one to reach the second floor again.
2F
Grab the HP Up, jump the ledge, and take the ladder back to 3F.
3F
Follow the trail, and take the first path to the north. Step around the hole to find a Rare Candy sitting behind the tall rocks, then jump down the hole to 2F one last time.
2F
Head west and climb down the stairway to find TM26 (Earthquake). Climb back up the stairs and take the ladder to return to 3F.
3F
Head east and jump the ledge. Use Rock Smash to break through the three brittle boulders in order to obtain TM79 (Dark Pulse). Before you leave the area with the three boulders, check that your team is ready for a fight; as soon as you approach the exit, your rival shows up for a battle.
When he leaves, you are free to head north to the exit. Next stop, the Pokémon League!
