Below is a summary of all the data I've collected on hidden Pokemon I encountered on Hoenn Route 103 (only shortly after having first met Wally). I encountered X of every species in the area so they all had the right search level, and then I ran around and looked at the info for any hidden Pokemon that popped up, scaring them away afterwards. For every search level, I looked at 500 hidden Pokemon.
The places where chances actually change are (apparently and reportedly) at search levels 5, 10, 25, 50, and 100, but I have a couple other search levels below as well on account of (on the one hand) a typo and (on the other) prudence. At the moment I don't really plan to check search levels like 9, 24, 49, and 99 (and 0), but since the 5/10/etc borders were given in the official guide book, I'm not terribly concerned either. I also might like to check, say, search level 200 and/or 1000, just to see if 100 is really the last change, but the chances of me doing that are also fairly small (though a little better than checking search level 9/etc).
Abbreviations used below are: HA = Hidden Ability, EM = Egg move, BL = Boosted level (+10 levels), X = none of the preceding.

Search level 4
This is a bit different mostly because, at search level 4, the DexNav's hidden Pokemon screen doesn't tell you potential (and figuring out would have been a massive pain, at best).
