Medium (Japanese: イタコ Itako) is a type of Pokémon Trainer that first debuted in Generation II games. They are depicted as old women with little ghosts floating around them in Generation II, and as old women with a large, lit candle in each hand in Generation IV. As their Japanese name suggests, they're based on blind female shamans from northern Japan who dress in a white kimono during their training for initiation (Itako). In the Japanese versions of Generation II's games, they are also shown carrying Buddhist prayer beads, which were removed in the Western versions due to Nintendo's policy on religious references.

They only appear in the Ecruteak Gym, where they specialize in Ghost-type Pokémon, and the Saffron Gym, where they specialize in Psychic-type Pokémon.
