When the player first arrives, the only thing to do is to explore the inner first floor of the cave. Many [[ice tile]]s are scattered around, causing players to fall through once they are stepped over twice. There is also slippery ice, causing players to continuously slide in the same direction they stepped in. After much navigating, {{hm|07|Waterfall}} can be obtained. Once the player uses the HM to scale the waterfall to the back inner cave, Lorelei of the Elite Four will be found confronting Team Rocket grunts. The player must help her to defeat them. After they are defeated, Team Rocket will flee to their base on [[Chrono Island]] and Lorelei will temporarily stay in her house on Four Island.

Four Island first appeared in A Beastly Cold Reception, where Red, Blue, and Bill hid inside Lorelei's house after she had narrowly helped them to escape from the Three Beasts of Team Rocket inside Icefall Cave. Soon after, the Three Beasts sent out a television broadcast, announcing that they would launch an attack across the Sevii Islands unless the Pallet Town Pokédex holders were turned over to them. As such, Red, Blue, and Lorelei headed out to stop them in the following chapter.
